If you wish to apply for enrollment for your child for the 2025-2026

school year, please complete, and return the following documents:

Required Health Form and  Emergency Contact Form.    Complete and return to Preschool.

Completed forms can be returned:

• Preschool Office, M-F, 9-11 am, (back of the Church) Summer Hours vary. Call ahead. 

• US mail directly to the address Darlene Adams Preschool Director, Preschool Express

• Drop off to the Church Office (front of building)   M-F 9-1 pm.

Enclose all paperwork/payment in an envelope marked Attention: Preschool Director, Darlene Adams, for processing.
